 The reporting phenomena is not limited to the affordable care act. Health insurance companies have been doing business this way for years. Your treatment and / or length of hospital stay are determined by what the insurer allows and practitioners get paid on the "correctness" of their billing. Unfortunately similar situations pervade all areas of our life due, in my opinion, to a business/ legal system that is so complex it defeats the purpose.
