 Not sure about private insurance companies but for public funded programs prof of citizenship is required (at least here in CA). Of course who knows if the documentation is valid. Counterfeit "papers" can be obtained in most major cities for a few hundred dollars. Of course in health care there is the law that no county hospital can turn anyone
away. Everyone gets "billed" but few bills are paid.
